Stewart Memorial Community Hospital (SMCH) in Lake City wants you to share your experiences with an outstanding member of their nursing staff. Nominations are now being sought for the DAISY award, which recognizes nurses who have made a profound difference in the lives of their patients. Nominations can be made by physicians, peers, patients, families and other staff members. Eligible nurses include those working at Stewart Memorial and at the McCrary Rost Clinics. Nomination forms are available at the hospital business office, at outpatient registration, at the front desk of any of the clinics and on the website at www.stewartmemorial.org. Forms are due back by May 1 to Chief Nursing Officer, Kari Jones, or to Administrative Assistant, Jodi Henkenius. There is also a mailing address for the return of nomination forms included below. The DAISY Foundation was started in 1999 when J. Patrick Barnes died at the age of 33 from complications of an auto-immune disease. The care he and his family received from the nurses inspired the creation of the foundation and the award.
