SMCH Asking For Input On Healthcare Needs Through Short Survey

Stewart Memorial Community Hospital (SMCH) of Lake City is seeking input to get a better understanding of the healthcare needs of the communities they serve. Officials are conducting a short 18-question survey that can be done online or on paper and returned to the hospital. The survey is designed to pinpoint the concerns people have about their personal health, the health of their family and their community. The identified goal is to discover the barriers community members are facing in becoming healthier. Results from the survey will help community leaders further develop the Health Improvement Plan, which was initiated in 2015. “The new survey will be used to validate the progress we’ve made in the past three years and will help us create future goals to focus on,” says CEO, Cindy Carstens. Several different programs have been developed based on feedback from previous surveys and include a Diabetes Prevention Program and State Innovation Models to help Calhoun County individuals with medical challenges. Everyone in the SMCH service area is invited to participate in the survey and can do so by following the link included below.
