Stewart Memorial Community Hospital (SMCH) in Lake City and the McCrary Rost Clinics have opened up nominations for the 2021 DAISY Foundation award, to be presented to an outstanding nurse selected by the community. This person is someone who goes above and beyond in providing excellent every day care to patients and their families alike. Nominations can be made by peers, physicians, patients, families, friends and other staff members and can include nurses from the hospital or any of the McCrary Rost Clinic locations. The DAISY Award originated in 1999 when J. Patrick Barnes died at the age of 33 due to complications from Idiopathic Thrombocytopenic Purpura (ITP), a little known but not uncommon auto-immune disease. His family was moved by the care Patrick and they received from his nurses, and developed this unique way of thanking those who continue to make profound differences in the lives of others. Nomination papers can be picked up at the Stewart Memorial business office, outpatient registration, at any McCrary Rost Clinic and can be downloaded off the website at www.stewartmemorial.org. Forms are due back to Chief Nursing Officer, Ann Lengeling, or to Administrative Assistant, Jodi Henkenius, by April 5th. Mailing instructions are included below.
